If you would like a file stamped copy of your proof of claim mailed to you
include a self addressed stamped envelope, the original proof of claim, and two
copies of the proof of claim to the address listed above.
The Last day to file claims is July 21, 2008. Please submit all
claims to the bankruptcy court by this date.
